Our easy to use ORDER FORM will help place your order at BarCode Base. Filling it out completely and accurately will ensure that your order is shipped as quickly as possible.

Placing an order offline with BarCode Base is as simple as:

1. Open the BarCode Base ORDER FORM located HERE.
2. Fill out the form completely and accurately as possible.
3. Either fax your order to us at 1-408-288-5354, e-mail to sales@barcodebase.com or send by U.S. Mail to:

PLEASE NOTE: The following information is absolutely REQUIRED on the form. Our ability to fill and ship your order will be compromised if you do not provide all of the following:

• Name and Shipping Address
• Email Address
• Credit Card Number
• Credit Card Expiration Date
• Credit Card CVV2
• Name as it appears on Credit Card and Billing Address where credit card statement is mailed to

All fax and mail orders will be shipped via our shipping program. Details of this program can found HERE. PLEASE NOTE: BarCode Base does not ship Internationally or to P.O. Boxes.

You will be notified by email when a Customer Service Representative has entered your order in our system.

You will also receive a shipment confirmation email when your order is processed by our shipping department.
